The principle: it is done when the anatomy can actually be improved

The non-surgical nose job is proposed when it is clear that the anatomy of the patient's nose can genuinely be improved with a filler injection alone. It is an "additive" assessment: the filler adds volume, it never removes it. This is why the best results are achieved on anatomically suitable noses — small bumps to be evened out by filling the areas above and below, post-traumatic depressions, modest asymmetries of the dorsum, a lack of tip projection — not on reductive changes.

When the filler makes it worse instead of better

There is a category of cases in which the non-surgical nose job simply would not be able to improve the aesthetics of the nose, and could even make them worse. The most typical case is the "prominent" nose, already large in itself: if filler is added to eliminate a small bump, the nose becomes even more prominent. Or it may take on a profile like a "Greek nose" — a straight, vertical dorsum, with the tip in continuity with the forehead — which the patient does not like. In these cases the only correct option is plastic surgery. It is important to say this honestly at the preliminary consultation, before the injection, not after.

A patient aged 30-35: non-surgical nose job or surgical rhinoplasty?

This is a question that comes up frequently in the clinic, and there is only one answer: what can we achieve with the non-surgical nose job? This is explained based on the patient's individual anatomy, with a mirror or with a standardised photographic study. At that point the patient decides with full awareness: "yes, this type of correction suits me" or "no, what I really dislike about my nose would not be corrected by the non-surgical nose job". In the first case we proceed with the filler; in the second we honestly recommend surgical rhinoplasty. A young age is not in itself an argument against surgery: it is the case-by-case assessment that guides the choice.

The risk we talk about openly: vascular necrosis

The risks of the non-surgical nose job are in line with those of other fillers, with one additional specificity linked to the area: the nose is very rich in arterial vessels. The most important risk is the possibility of injecting filler into an artery, a rare but possible event, with the potential consequence of tissue necrosis. There are two ways to manage this eventuality safely: knowing the vascular anatomy in depth — one of the reasons the non-surgical nose job must be performed by a physician, not by an aesthetic operator — and waiting a few minutes at the end of the session, carefully observing the skin for any areas of blanching, the typical sign of a vascular event. Should the skin "blanch", there is an immediate management protocol that includes injecting hyaluronidase to dissolve the product and restore the blood supply. It is a standard procedure in medical centres, impossible to guarantee outside of them.
